Actilyse 50mg injection converts plasminogen enzyme to plasmin, which helps in breaking down of fibrin present in blood clots, which leads to restoration of blood flow. So it is used in acute ischemic stroke; it dissolves clots, minimizing damage. 

1. Anticoagulants like Heparin or Warfarin (Life threatening)
   Using alteplase with blood thinners can greatly increase the risk of severe or uncontrolled bleeding. Such combinations are used only under strict hospital monitoring.

2. Antiplatelet Medicines such as Aspirin or Clopidogrel (Severe)
   These drugs can enhance the bleeding risk when given with alteplase. Doctors carefully weigh benefits and risks before combining them.

3. Direct Oral Anticoagulants like Apixaban or Rivaroxaban (Severe)
   Recent or ongoing use may raise the chance of intracranial or gastrointestinal bleeding. Alteplase is usually avoided in these situations.
