NIRMATRELVIR,RITONAVIR
Background and Date of Approval
Nirmatrelvir with Ritonavir is an oral antiviral combination developed for the treatment of COVID-19, especially in high-risk individuals.
This combination was created to block viral replication early in the infection and prevent progression to severe disease.
The therapy received its first major regulatory authorisation under Emergency Use Authorisation (EUA) by the U.S. FDA in December 2021.
Following this, it was approved or granted emergency authorisation by regulatory agencies worldwide, including:
European Medicines Agency (EMA) – 2022
UK MHRA – December 2021
India (DCGI) – early 2022
Since then, Nirmatrelvir + Ritonavir has become a key outpatient treatment for individuals at higher risk of developing severe COVID-19, reducing hospitalisation and mortality when given early.
Uses
Nirmatrelvir (Nirmatrelvir + Ritonavir) is used to:
Treat mild to moderate COVID-19 in adults at high risk of complications
Prevent progression to severe illness, hospitalisation, or death.
Reduce viral load and shorten symptom duration.
Provide early antiviral control in immunocompromised or vulnerable patients.
It must be started within 5 days of symptom onset for best results.
How Nirmatrelvir Works
Nirmatrelvir works through a dual-component antiviral mechanism:
1. Nirmatrelvir – Main Antiviral Agent
Nirmatrelvir inhibits the SARS-CoV-2 3CL protease, an enzyme essential for viral replication.
Blocking this enzyme prevents the virus from maturing and multiplying inside the body.
2. Ritonavir – Pharmacokinetic Booster
Ritonavir does not act directly against COVID-19.
It slows down the breakdown of Nirmatrelvir by inhibiting CYP3A4 enzymes in the liver.
This increases Nirmatrelvir’s concentration and ensures effective antiviral levels throughout treatment.
Drug Administration and Dosage Information
Nirmatrelvir is taken orally, usually as two components together:
Nirmatrelvir tablets
Ritonavir tablets
It is taken twice daily for 5 days.
Typical regimen for adults:
300 mg Nirmatrelvir + 100 mg Ritonavir every 12 hours for 5 days
(The exact composition depends on the manufacturer’s tablet strength.)
