SOTATERCEPT-CSRK
Background and Date of Approval of Sotatercept-CSRK
Sotatercept-CSRK is a novel biological medicine developed to treat pulmonary arterial hypertension (PAH), a serious condition where the blood pressure in the arteries of the lungs becomes abnormally high. This can strain the heart and gradually reduce its ability to pump blood effectively.
Unlike traditional therapies that primarily target blood vessel relaxation, Sotatercept-CSRK targets a different biological pathway. It helps restore balance in the signalling processes that control blood vessel growth and repair, which are often disrupted in PAH.
Sotatercept-CSRK received its first regulatory approval in March 2024 from the United States Food and Drug Administration (FDA) for use in adults with pulmonary arterial hypertension to improve exercise capacity and delay disease progression. This approval marked an important advancement in PAH treatment, as it introduced a first-in-class therapy targeting the underlying disease mechanism rather than only managing symptoms.
Uses
Sotatercept-CSRK is used for the following conditions:
Treatment of pulmonary arterial hypertension (PAH) in adults
Improvement in exercise ability and physical activity
Reduction in symptoms such as breathlessness, fatigue, and chest discomfort
Slowing the progression of PAH
Lowering the risk of hospitalisation due to worsening condition
It is often used alongside other PAH medications for better disease control.
How is Sotatercept-CSRK administered?
Sotatercept-CSRK is given as a subcutaneous injection, meaning it is injected into the skin.
It is usually administered once every 3 weeks.
The injection is commonly given in the abdomen, thigh, or upper arm.
Treatment is started and supervised by a healthcare professional experienced in managing pulmonary arterial hypertension.
The dose is individualised based on body weight and blood test results, especially haemoglobin and platelet levels.
In some cases, after proper training, patients or caregivers may be allowed to administer the injection at home.
Regular monitoring is important during treatment to ensure the medicine is working effectively and to reduce the risk of side effects.
Warnings
Sotatercept-CSRK should be used with caution as it may cause certain serious side effects. Patients must be carefully monitored during treatment.
Increased haemoglobin levels: The medicine can raise red blood cell levels, potentially increasing the risk of blood clots. Regular blood tests are required.
Risk of bleeding: Some patients may experience bleeding episodes, including nosebleeds and more serious complications.
Thromboembolic events: There is a potential risk of blood clots forming in veins or arteries, which can lead to serious conditions like stroke or pulmonary embolism.
Embryo fetal toxicity: Sotatercept-CSRK may harm an unborn baby. It should not be used during pregnancy.
Allergic reactions: Though uncommon, serious hypersensitivity reactions can occur and require immediate medical attention.
Precautions
Before and during treatment with Sotatercept-CSRK, certain precautions should be followed to ensure safety and effectiveness:
Regular monitoring: Blood counts, especially haemoglobin and platelet levels, should be checked before and during treatment.
Pregnancy prevention: Women of childbearing age should use effective contraception during treatment and for a specified period after stopping the medicine.
Breastfeeding: It is not known whether the drug passes into breast milk, so breastfeeding should be avoided unless advised by a doctor.
History of clotting disorders: Patients with a history of blood clots should inform their doctor, as this medicine may increase risk.
Liver function monitoring: Patients with liver conditions may require closer observation.
Dose adjustments: Based on laboratory results and side effects, doctors may adjust the dose or temporarily stop treatment.
Side Effects
Like all medicines, Sotatercept-CSRK can cause side effects.
Common Side Effects
Headache
Nosebleeds
Skin rash or redness
Diarrhoea
Dizziness
Visible small blood vessels on the skin
These effects are usually mild and temporary.
Serious Side Effects

Increased red blood cell count (erythrocytosis)
Low platelet levels (thrombocytopenia)
Risk of blood clots due to thickened blood
Seek immediate medical help if you experience:
Unusual bleeding or bruising
Black stools or blood in stools
Severe headache or vision problems
Sudden weakness or confusion
Expert Tips
